General Requirements


  1. Appointments. Appointments may be secured at:

  2. Application forms. Application forms may be downloaded here. For passport applicants, a duly filled-out passport application form will be generated upon appointment confirmation through the Global Online Appointment System at https://www.passport.gov.ph.
  3. Fees. Schedule of fees may be downloaded at here.
  4. Valid IDs. For a list of valid IDs, please see refer to this page.
  5. Documents requiring apostille:

    1. Documents issued in the Philippines to be used in Korea need to be apostilled in the Philippines. https://co.dfaapostille.ph/appointment/Account/Login.
    2. Documents issued by the Korean Government to be used in the Philippines must have an English translation, notarized, and apostilled by the Korean Ministry of Foreign Affairs at 6F Seocho-gu Diplomatic Center, 2558 Nambusunhwan-ro, Secho-gu, Seoul (Tel. No. 02-733-2114).
  6. Foreign documents. Other foreign documents must have an English translation and apostilled by the relevant agencies.
  7. Pictures. All pictures must be taken against a white background and without glasses, hat/cap or other accessories.
  8. Details. Names and/or details in all documents must be consistent. Please make sure that the order of names and spelling are correct.
  9. Signature page of the passport. Please sign the signature page of your Philippine passport before photocopying it.

NOTARIAL


Acknowledgments and Notarials of Affidavits, Special Power of Attorneys

Original and photocopy of the following:

  • Filled-out Application form
  • Valid Passport
  • Identification Certificate for dual citizens
  • Document to be acknowledged/notarized

Certificate of Natural Born

Original and photocopy of the following:

  • Filled-out Application Form
  • Child’s Philippine Passport
  • Mother’s Philippine Passport (with signature)
  • Child’s NSO/PSA Birth Certificate
  • Parents’ NSO/PSA Marriage Certificate
  • Authorization letter from the Korean Immigration (with English Translation)
  • Child’s Basic Certificate 기본증명서상세 (with English translation)
  • Child’s Certificate of Family Relation/Registry 가족관계증명서상세 (with English Translation)

Certificate of Philippine Driver’s License

Original and photocopy of the following:

  • Filled-out Application Form
  • DFA Apostilled Certification from LTO
  • Valid Driver’s License (not expired)
  • Passport
  • Valid Alien Registration Card (for Filipinos) and/or Korean ID (for Koreans)
  • Photocopy of Exit and Entry Immigration Stamp in the passport

Embassy ID

Original and photocopy of the following:

  • Filled-out Application Form
  • Valid passport
  • 2 pcs. 1x1 size picture (front view)

NBI Clearance

Original and photocopy of the following:

  • Filled-out Application Form
  • Valid passport
  • 1 pc. 2x2 size picture (front view)
  • NSO/PSA Marriage Certificate for married Filipina who has changed her surname
Note: The NBI form is not a consular document. However, the Philippine Embassy assists Filipinos who are applying for NBI clearance in terms of fingerprinting on the card form. Upon completion of the requirements, the applicant must send the form, together with an authorization letter, to his/her authorized representative in the Philippines, who will process the clearance with the NBI on your behalf.

DUAL CITIZENSHIP & RENUNCIATION


Philippine Citizenship Retention and Reacquisition (RA 9225)

Original and three (3) photocopies of the following:

  • Filled-out Application Form
  • NSO/PSA Birth Certificate
  • NSO/PSA Marriage Certificate
  • Philippine passport or 2 Philippine government-issued ID
  • Affidavit of Renunciation, if any
  • Certificate of Naturalization (with English translation), notarized
  • Foreign Passport and Korean ID
  • English Certificate of Family Registry
  • English Basic Certificate 기본증명서 상세, notarized
  • 6 pcs. 2x2 size picture (front view)
  • 1 pc. 2x2 size picture (left-side view)
  • 1 pc. 2x2 size picture (right-side view)

Philippine Citizenship Retention and Reacquisition (RA 9225) for Dependents below 18 years old

Original and three (3) photocopies of the following:

  • Filled-out Application Form
  • NSO/PSA Birth Certificate or typewritten Birth Certificate of the Child from the hospital with the correct information (with English Translation)
  • Foreign passport
  • 4 pcs. 2x2 size picture (front view)
  • 1 pc. 2x2 size picture (left-side view)
  • 1 pc. 2x2 size picture (right-side view)
  • Dual Certificate of the parent

Renunciation of Philippine Citizenship

Original and photocopy of the following:
