General Requirements
Appointments. Appointments may be secured at:
- Application forms. Application forms may be downloaded here. For passport applicants, a duly filled-out passport application form will be generated upon appointment confirmation through the Global Online Appointment System at https://www.passport.gov.ph.
- Fees. Schedule of fees may be downloaded at here.
- Valid IDs. For a list of valid IDs, please see refer to this page.
Documents requiring apostille:
- Documents issued in the Philippines to be used in Korea need to be apostilled in the Philippines. https://co.dfaapostille.ph/appointment/Account/Login.
- Documents issued by the Korean Government to be used in the Philippines must have an English translation, notarized, and apostilled by the Korean Ministry of Foreign Affairs at 6F Seocho-gu Diplomatic Center, 2558 Nambusunhwan-ro, Secho-gu, Seoul (Tel. No. 02-733-2114).
- Foreign documents. Other foreign documents must have an English translation and apostilled by the relevant agencies.
- Pictures. All pictures must be taken against a white background and without glasses, hat/cap or other accessories.
- Details. Names and/or details in all documents must be consistent. Please make sure that the order of names and spelling are correct.
- Signature page of the passport. Please sign the signature page of your Philippine passport before photocopying it.

PASSPORTS AND TRAVEL DOCUMENT
New Passport
Original and photocopy of the following:
- Filled-out Application Form
- PSA Birth Certificate (Report of Birth may be submitted within 1 year from its issuance for minors)
Passport Renewal
Original and photocopy of the following:
- Filled-out Application Form
- Photocopy of old passport
- PSA Birth Certificate for holders of green or brown passports
- PSA Documents (e.g., Marriage Certificate, Annotated Marriage and Birth Certificates, Death Certificate of Spouse) if name is to be amended
- Certificate of Naturalization or Affidavit of Election of PH Citizenship, if any
Passport Renewal of Minors
Original and photocopy of the following:
- Filled-out Application Form
- Photocopy of old passport
- PSA Birth Certificate of child
- PSA Marriage Certificate if only one parent is accompanying the child. If parents are unmarried, an Affidavit of Support and/or Consent executed by the Mother
- Passport of Filipino parents
Replacement of Damaged/Lost Passport
Original and photocopy of the following:
- Filled-out Application Form
- PSA Birth Certificate
- Photocopy of damaged/lost passport’s data page, if available
- Police Report for lost passport, with English translation
- Affidavit of Loss, notarized at the Embassy
- Any two PH-Government Issued ID with picture (e.g., SSS, Driver’s License, GSIS, PRC, OWWA)
Passport Extensions
Original and photocopy of the following:
- Filled-out application form
- Passport
- Confirmed flight itinerary
Passport Extensions for Seafarers
Original and photocopy of the following:
- Filled-out application form
- Passport of the applicant
- Passport of the representative
- Request letter from the Manning Agency
- Authorization letter issued by the seafarer
Travel Document
Original and photocopy of the following:
- Filled-out Application Form
- 4 pcs. passport-size picture
For lost or mutilated passports:
- Expired, lost or mutilated passport, if available
- Old Travel Documents, if available
- Affidavit of Lost/Mutilated passport
- Police Report with English Translation, if available
- NSO/PSA Birth Certificate and PH Government-issued IDs under true name, for those with passports using assumed identities
- NSO/PSA Marriage Certificate for married women
For seafarers:
- Seaman’s Book
- Guarantee Letter from Shipping Company
- Authorization letter (apostilled by the DFA if issued in the Philippines)
- Photocopy of representative’s valid passport with signature
For those with urgent medical concerns:
- Medical Certificate for ATN cases
For detained applicants:
- Detention Order from Korean Immigration for detained applicants
For Minor applicants:
- NSO/PSA Birth Certificate of the child
- NSO/PSA Marriage Certificate and valid passports of the parents
For Minor applicants who are not traveling with their parents
- Affidavit of Parental Consent to Travel
- Valid passport of the child’s guardian
- Ticket/flight itinerary going back to the PH of both child and traveling guardian
- DSWD endorsement if no parent will be accompanying the child to the Philippines
